How can our office obtain policy benefits?
Benefit requests must be submitted in writing on medical office letterhead and responses are only provided in writing, either by fax or mail. Requests should include:
• Patient name
• Insurance certificate number
• Diagnosis
• Procedure

What services require pre-certification or pre-approval?
Pre-certification is the responsibility of the insured and is required for any Inpatient, 23-hour observation, or overnight hospital stays. To pre-certify a hospital stay, please call 800-245-3005.
Pre-Approval is the responsibility of the insured and is required for the following types of services:
• Home health care
• Injectable medications, in a physician’s office or at home
• Durable medical equipment
• Orthotics and prosthetics
• Transplants
We have contracted with special suppliers for preferred pricing. To begin the pre-approval process for these types of services or to obtain a contracted supplier, please call Case Management at 800-371-9622, Extension 2782 or you may fax the request to Attention: Case Management at fax number 309-346-8265.
Chemotherapy has specific limitations, though it does not require pre-approval. Please contact Case Management at 800-371-9622, Extension 2782 for additional information regarding chemotherapy.
